Remembering the question after an appointment is such an awkward moment

An appointment can move quickly, especially when there are forms, timings and practical instructions to follow. The question that seemed obvious beforehand may only return once the conversation is over. This discussion is about communication: what helps people keep track of what they wanted to ask, and what makes it easier to ask for clarification? Written notes, a clear summary or an invitation to pause might all play a part. Please keep personal medical details out of replies; experiences of the conversation itself are enough.

Questions about where to go next or how to arrange a follow-up can be easy to miss. Clear written instructions would be useful examples.
